Required Forms

The following forms are required for the MA program and MUST be submitted within 30 days of enrollment.

1- Medical Assistant Online Data Sheet
2- Clinical/Classroom Training or Externship
3- Medical Program Acknowledgement of Responsibility
4- Medical Assisting Program Technical Standards Requirement Form
5- Medical Assistant Online Required Documentation and Equipment Form
6- Medical Assistant Online Preceptorship Training Form
7- Medical Insurance Acknowledgement Form
8- Hepatitis B Immunization Documentation Form
9- Pregnancy Form
10- Statement of General Health
11- Preceptorship/Externship Affidavit

In addition to the forms listed above the following are also required.

• Two head shot photos, for your student identification card, approximately 2"x2"
• A copy of your driver license
• A copy of your health insurance

    ***Students are required to have health insurance coverage during preceptorship and externship
